Do NOT include patient names, diagnoses, medical record numbers, or other PHI. This website and its forms are not intended for the transmission of PHI or clinical records.
We collect the minimum information necessary to respond to your request. Submissions are retained only as long as needed for business operations or legal obligations and are periodically reviewed and deleted.
Access to submitted information is restricted to authorized personnel. We use standard security controls (including HTTPS) to protect data in transit.
If PHI is inadvertently submitted through this website, we will take reasonable steps to delete or redact such information and not further process it.
